TARGETED CASE MANAGEMENT-TRANSITION COORDINATION (TCM-TC)
We are a registered Targeted Case Management Agency and Home & Community-Based Service (HCBS) provider. We provide Transition Coordination Services through the Colorado Department of Health Care Policy and Financing (HCPF) State Plan and the Centers for Medicare & Medicaid (CMS). We work with individuals who may be in long-term care facilities such as nursing homes or intermediate care facilities for people with intellectual disabilities. Our agency collaborates with the individual, their chosen support groups, and a team of specialists to explore less restrictive living options, including assisted living programs or independent living. We support individuals in navigating the complex system and simplify it to ensure their success.
LIFE SKILLS TRAINING
Our professionals work with Medicaid-qualified individuals; to individualize training designed and directed with them to develop and maintain their ability to independently sustain physically, emotionally, socially, and economically in the community.


PEER MENTIORSHIP

Is a service provided by peers to Medicaid participants who have transitioned into the community from a long-term care facility. This service promotes self-advocacy, instructing and advising on issues and topics related to community living, describing real-world experiences as examples, and modeling successful community living and problem-solving.

HOUSEHOLD SETUP & EXPENSES
Household setup & expense is a one-time, non-recurring service that allows a qualified member who is transitioning to the community to establish a basic household setup.
